Managed Thesis, Laboratory Work and Assistant Seminars:
Yurt dışında araştırma yaparak değerlendiridiğim bir yılın ardından, kendi kliniğimde yine kendi olanaklarımızla kurduğum Embrioloji-Histoloji laboratuarında tavuk yumurtasında embrio çalışmalarını başlattım ve orta hat kapanma defektlerinin embriolojisini çalışmaya başladım. Bu cümleden olarak verapamilin nörilasyona etkisi ilk olarak yapılan çalışma olarak ulusal kongrede sunuldu.
Selçuk Üniversitesi Tıp Fakültesi Nöroşirürji Ana Bilim Dalından Yrd.Doç.Dr.Önder Güney’in, laboraturamızda yaptığı çalışma ile, diazepamın orta hat kapanmasına olan etkisi araştırılmış ve denetleyici olarak katıldığım araştırma Türk Nöroşirürji Dergisinde yayımlanmak üzere kabul edilmiştir.
Kliniğimizde yetişen Dr.Çağlar Berk’in “Subaraknoid kanama geçiren hastalarda iv asetozolamid verilmesi öncesi ve sonrası vazoreaktivitenin SPECT ile değerlendirilmesi – Prospektif klinik çalışma” isimli uzmanlık tezi,
Dr.Gökalp Silav’ın “Orta hat kapanmasında NMDA reseptörlerinin ve bu reseptörlerin blokajının etkisi” isimli uzmanlık tezi,
Dr.Varol Aydın’ın “Tavşanlarda deneysel tethered cord modeli” isimli uzmanlık tezi benim tarafımdan yönetilmiştir.
CBÜ de çalışmaya başladıktan sonra, burada da bir tavuk yumurtası ve embriyoloji laboratuarı kurarak çalışmalarıma devam ettim. Bu laboratuarda sadece CBÜ nöroşirürji asistanları değil, Manisa dışındaki çeşitli merkezlerden gelen nöroşirürji asistanlarının ihtisas tezleri de deneysel olarak gerçekleştirilmiştir.

Abstract
PurposeTethered cord syndrome (TCS) typically presents with urologic symptoms and abnormal imaging findings. However, some patients present with normal conus medullaris level and filum terminale appearance on MRI. This research seeks to assess the intended surgical results in this particular group of TCS patients who do not present with urologic complaints, under the premise that the surgical approach goes a long way in preventing the onset of urologic abnormalities.MethodsThis retrospective study included 59 operated patients with tethered cord syndrome who had a normal level terminating conus medullaris and a normal looking filum terminale without urologic symptoms. Of these patients, 38 were female and 21 were male. All patients underwent somatosensory-evoked potentials (SSEPs), and magnetic resonance imaging (MRI). The surgical technique used was flavotomy, which involves cutting the filum terminale without performing a laminectomy.ResultsThe study population had a mean age of 22.5 years (SD = 13.2). During the mean postoperative follow-up period of 2.5 years, none of the patients developed urinary incontinence. Preoperative SSEP abnormalities included conduction block in 39 patients (66.1%), low amplitude in 12 patients (20.3%), and delayed N22 wave latency in 8 patients (13.5%). The surgical procedures were completed without morbidity or mortality, and all patients showed significant postoperative improvement in SSEP parameters.ConclusionOur results indicate that even though the filum terminale might have a normal looking MRI, TCS can also occur due to some potential microscopic or structural abnormality. The study proves SSEP to be useful in TCS diagnosis and it also proposes that if surgery is done early before any urologic complaints arise, chances of their onset will be minimized. Such findings support the view that surgical measures should be entertained in symptomatic patients with abnormal SSEP but normal MRI.

Abstract
AIM: To evaluate the neuroprotective effects of deocanthal OC in a rat model of traumatic brain injury (TBI).
MATERIAL and METHODS: Twenty-six adult male, Wistar albino rats were used. The rats were divided into 4 groups. Group 1 was the sham group (n=5). Group 2 was the trauma group (n=5) where rats were treated with 10 mg/kg saline intraperitoneally (IP) twice a day. Groups 3 and 4, rats were treated with 10 (group 3, n=8) or 30 (group 4, n=8) mg/kg OC IP twice a day. For each group, brain samples were collected 72 hours after injury. Brain samples and blood were evaluated with histopathological and biochemical methods.
RESULTS: Histopathological evaluation revealed a significant difference between Group 2 and Group 4. Biochemical findings demonstrated that the oxidative stress index was highest in Group 2 and lowest in Group 4.
CONCLUSION: OC has a protective effect on neural cells after TBI. This effect is achieved by reducing oxidative stress and apoptosis.

Abstract
In this study, three different doses of LCM (0.12, 0.5, and 1.60 mg in 0.18 mL) were applied under the embryonic disks of specific pathogen-free Leghorn chicken embryos after a 30-h incubation. Incubation was continued for 80 h, at which time all embryos were evaluated macroscopically and microscopically.
There was growth retardation in all of the LCM-treated groups. Major malformations increased in a dose-dependent manner and were mostly observed in the supratherapeutic group.
Based on our data, LCM may cause growth retardation or major congenital malformations. Nevertheless, more extensive investigations of its reliability are needed.

Abstract
AIM: The aim of this study was to describe the results of surgery performed in a group of adult patients with tethered cord syndrome with their outcomes.
MATERIAL and METHODS: This retrospective study included 56 patients. There were 38 females and 18 males. All patients were older than 18 years.
RESULTS: The mean age at referral was 36 years and 1 month. The mean follow-up period was 10 months 27 days. 95% of all patients with back and leg pains improved and 5% remained the same. Three patients with motor deficits remained the same in the postoperative period. Of the 16 patients with urological complaints, 10 improved, 5 unchanged and 1 patient died in the postoperative first day due to pulmonary embolism.
CONCLUSION: The syndrome of tethered cord may be a situation to be treated even in the elderly in case of normal level conus medullaris and filum terminale with a normal appearance as well as a low-lying conus and thick filum. To prevent overlooking the diagnosis of tethered cord and/or unnecessary spinal surgeries, the tethered cord syndrome should be remembered in the differential diagnosis list in the presence of back and leg pains, neurological deficits or urological complaints.

Abstract
Background: Tethered cord syndrome (TCS) is a progressive clinical entity that arises from abnormal spinal cord tension. Scoliosis may be a unique symptom in TCS. The aim of this study is to investigate prognosis after releasing the filum terminale in scoliosis due to TCS with/without findings in magnetic resonance imaging (MRI) and to draw attention to the importance of somatosensorial evoked potentials (SSEP) on the differential diagnosis of idiopathic scoliosis versus scoliosis due to TCS with normal appearance of filum terminale and conus medullaris. Materials and Methods: Eleven female and seven male patients with progressive scoliosis were included in the study. They were evaluated radiologically, SSEP and urodynamical studies. Preoperative and postoperative anteroposterior full spine X-rays were obtained for measuring the Cobb's angle. MRI was performed in all cases for probable additional spinal abnormalities. All patients underwent filum terminale sectioning through a L5 hemilaminectomy. The resected filum terminale were subjected to histopathological examination. Results: The mean Cobb angle was 31.6 degrees (range 18 degrees u45 degrees). Eight patients (44.45%) had a normal appearance of filum terminale and normal level conus medullaris in MRI, but conduction delay and/or block was seen on SSEP. In the histopathological examination of filum terminale dense collagen fibers, hyaline degeneration and loss of elastic fibers were observed. Postoperatively none of the patients showed worsening of the Cobb angle. Three patients showed improvement of scoliosis. Conclusion: In TCS presented with scoliosis, untethering must be performed prior to the corrective spinal surgery. Absence of MRI findings does not definitely exclude TCS. SSEP is an important additional guidance in the diagnosis of TCS. After untethering, a followup period of 6 months is essential to show it untethering helps in stopping the progress of the scoliotic curve. In spite of non progression (curve stopped lesser than 45 degrees) or even improvement of scoliosis, there may be no need for major orthopedic surgical intervention.

Abstract
Background/aim: Bone marrow-derived mesenchymal stem cells (BMSCs) possess self-renewal characteristics that distinguish them from other cell types. Recent studies have focused on the effects of conditioned medium (CM) that includes the extracellular matrix. Here we examined the neuroprotective effects of BMSCs and CM on damaged neuroblastoma cells.
Materials and methods: The cells were divided into five groups: 1) healthy controls, 2) damaged cells alone, 3) damaged cells treated with BMSCs, 4) damaged cells treated with CM, and 5) damaged cells treated with both BMSCs and CM. Neuroprotective effects were then evaluated based upon the levels of oxidative stress, antitransforming growth factor beta 1 (anti-TGF beta 1) production, and apoptosis.
Results: Significant differences were observed between healthy controls and damaged cells (P < 0.001), as well as between damaged cells and those treated with BMSCs alone (P < 0.05), CM alone (P < 0.05), and both BMSCs and CM in combination (P < 0.01). Among the treated groups, the strongest neuroprotective effects were seen in cells treated with both BMSCs and CM.
Conclusion: These results show that both BMSCs and CM exhibit neuroprotective effects in damaged neuroblastoma cells. The strongest benefits were seen following treatment with both BMSCs and CM.

Abstract
Ventriculoperitoneal shunt (VPS) insertion is a common neurosurgical procedure for the treatment of hydrocephalus. Constipation can play an important role in shunt failure either by mechanically blocking drainage from the peritoneal end of the VPS catheter or by increasing intra-abdominal pressure. The present report describes a 2-year-old boy with shunt-dependent hydrocephalus who presented with signs of shunt dysfunction. Medical history with detailed physical examination determined the constipation. Before shunt revision, a pediatric fleet enema was administered to possibly relieve the constipation and he had good evacuation of the bowel contents. Following the enema, his continuous crying resolved and ultimately disappeared within a few hours. Brain computed tomography scans performed 4 days later showed a reduction in ventricular size.

Abstract
Split cord malformations (SCMs) are rare congenital anomalies of the vertebrae and the spinal cord. Tethered cord syndrome (TCS) is a clinical condition of various origins that arises from tension on the spinal cord. Radiographic findings may include and/or associate split cord malformations and the other neural tube defects. However, the spinal cord can even be tethered by a filum terminale with normal appearance and normal level conus medullaris in magnetic resonance imaging (MRI). The aim of our study is to show whether SMC patients with normal or abnormal MRI findings had all histological abnormal filum terminale and also to show that the standard SCM repairing operation without cutting filum will not achieve total release.
We have reviewed 33 SCM patients between July 2005 and December 2013. They were operated by adding untethering procedure of filum terminale following standard surgical intervention, and a part of the filum was taken for histopathological examination even though MRI did not show the presence of abnormality of filum terminale.
We found that abnormal filum terminale with a normal appearance may had dense collagen fibers, wide and numerous capillaries, and hyaline formation, while normal filum terminale is a mixture of collagen fibers and blood vessels. We did not obtain positive Verhoeff elastic fiber staining. The elastic fibers had disappeared in all fila terminalia, except control cadaver group.
Our results showed that all fila of SCM patients had loss of elastic fibers and increased of hyalinization, which means loss of elasticity of filum terminale. Less severe traction may remain asymptomatic in childhood and present with neurological dysfunction later in life. For this reason, surgical procedure of SCM patients including releasing of filum terminale seems more beneficial for the patients and be better for long term.

Abstract
AIM: The Chiari Malformation I (CM I) and the tethered cord syndrome (TCS) are both congenital abnormalities whose mechanisms are still not fully understood. The association of CM I and TCS has been reported only a few times previously.
MATERIAL AND METHODS: This retrospective study included 7 patients who were diagnosed with CM I and TCS, managed by cutting of the filum terminale.
RESULTS: The mean follow-up period was 21 months and 28 days. Although all patients underwent an untethering surgical procedure by cutting the filum terminale only, all patients reported significant early postsurgical resolution of CM I symptoms and symptoms related to TCS as well.
CONCLUSION: Patients with symptomatic CM I, even if lumbar MRI is normal and the patient asymptomatic for TCS, may have tethered spinal cord at the same time. It seems it would be worthwhile to investigate CM I patients for occult TCS with spinal somatosensory evoked potentials.

Abstract
Congenital dermal sinus tract is a rare entity which lined by epithelial cells and can end anywhere between subcutaneous planes to thecal sac. These tracts may be accompanied with other pathologies such as lipomyelomeningocele, myelomeningocele, split cord malformation, tethered cord, filum abnormality and inclusion tumors and treatment includes resection of tract with intradural exploration. The authors review their experience with 16 cases. Clinical, radiological appearance and treatment of these lesions discussed with literature review.

Abstract
AIM: The objective of this study is to examine the effects of radiation of mobile phones on developing neural tissue of chick embryos.
MATERIAL and METHODS: There were 4 study groups. All Groups were placed in equal distance, from the mobile phones. Serial sections were taken from each Group to study the neural tube segments.
RESULTS: The TUNEL results were statistically significant (p<0.001) at 30 and 48 hours in the third Group. We found low Bcl-2 levels partly in Group 4 and increased activity in Group 3. Caspase-3 was negative in the 48 and 72 hours in the Control Group, had moderate activity in the third Group 3, weak activity in the 48 hour, and was negative in the 72 hour in other groups. Caspase-9 immunoreactivity was weak in Group 1,2 and 3 at 30 hours and was negative in Group 1 and 4 at 48 and 72 hours. Caspase-9 activity in the third Group was weak in all three stages.
CONCLUSION: Electromagnetic radiation emitted by mobile phones caused developmental delay in chick embryos in early period. This finding suggests that the use of mobile phones by pregnant women may pose risks.

Abstract
Congenital cervical spinal anomalies are relatively common and can be seen in upper and lower cervical regions. However, hypertrophy of the lamina and spinous process of cervical vertebra is extremely rare. Here we reported an 11-year-old girl with unilateral hypertrophy of the lamina and spinous process of C5 vertebra coexistence with Klippel-Feil syndrome, occipitalization of atlas and atlantoaxial congenital fusion. Because of cosmetic anomaly patient underwent surgery and spinous process excised. To the best of our knowledge, this coexistence of congenital cervical bony anomalies in a child has not been reported previously in English literature.

Abstract
AIM: Transforming growth factor beta (TGF-beta) and Smads control intracellular signaling pathways in neurulation. Although previously reported similar experimental animal studies, the aim of this human study is to investigate the expression of TGF-beta (1,2,3) and Smads (1,2,3,6,7) in aborted human fetuses with myeloschisis.
MATERIAL and METHODS: Twelve human fetuses with neural tube defect were obtained. They were stained with antibodies against TGF-beta 1, TGF-beta 2,TGF-beta 3, Smad (1,2,3), Smad 6 and Smad 7 using the indirect immunohistochemical technique.
RESULTS: We noted mild immune reactivity of TGF-beta 1 and TGF-beta 2 in the open neural plate, motor neurons and surrounding tissue. Strong immune reactivity of TGF-beta 3 was shown in only open neural plate and surrounding tissue. Immunoreactivity of all Smads noted negative except Smad7.
CONCLUSION: These results suggested at the site where the neural tube failed to close, TGF-beta 1,2 and Smads 1,2,3,6 do not continue their activity and decrease with internal timing of embryonic development. Additionally ectodermal layers are considered by embryo as"not closed wound" and TGF-beta 3 activity may be an effort to repair the failed closure.

Abstract
Hydatid disease is a parasitic infection affecting the brain in about 2% of the cases. Brain involvement is most commonly observed in children. Here, we report a 13-year-old male patient who presented with headache, nausea, and vomiting. Before cranial computed tomography (CT) was performed, the patient had generalized epileptic seizures. He was disoriented, and had anisocoria with dilatation of the right pupilla. CT showed a cystic lesion of 10-cm diameter in the right temporoparietal region that had caused a shift of the midline structures to the contralateral side; an urgent operation was performed as there were signs of midbrain herniation.

Abstract
AIM: To investigate the apoptotic and molecular effects of glycogen synthase kinase-3 (GSK-3) in glioblastoma multiforme (GBM).
MATERIAL and METHODS: Human primary glioblastoma cell line (U-87 MG) and the human fetal glial cell line (SVGp12) were used. The cells were exposed to the different doses of GSK inhibitor for 24, 48 and 72 hours. Induction of apoptosis was assessed by DNA fragmentation (TUNEL) assay. EGFR and NF-kB expression was evaluated by immunofluorescence analyses.
RESULTS: GSK-3 inhibitor IX induced cytotoxicity and apoptosis in dose-dependent manner in GBM cells. Our results indicated that GSK-3 inhibitor IX induces apoptosis, resulting in a significant decrease in the expression of NF-kB and EGF.
CONCLUSION: Inhibition through GSK-3 has been found promising in creating therapeutic management of GBM cells. Proliferation, differentiation, cell cycle regulation, and apoptosis are mechanisms that must be interpreted as a whole. Components associated with EGFR, NF-kB, and apoptosis affect the mechanism solely and collectively. Our collective data suggest that GSK-3 inhibitor IX inhibited cellular proliferation and induced apoptotic events by modulating EGFR and NF-kB expression in GBM cells. GSK-3 inhibition holds promise for the development of new approaches for the therapeutic management of GBM cells.

Abstract
AIM: The aim of this study is to demonstrate the effect of meloxicam in early stage chick embryos on neural tube development.
MATERIAL and METHODS: One hundred specific pathogen-free (SPF) chicken eggs were used to investigate the neurulation. SPF eggs were invastigated in four groups (n:25). All of the groups were incubated at 37.2 +/- 0.1 degrees C and 60 +/- 5% relative humidity for 30 hours, and an embryological development in the ninth stage as classified by Hamburger and Hamilton was obtained. In the end of the 30th hour, group A (control group) was administered 0.1 ml of saline (0.9% NaCl) in ovo and the other groups were administered meloxicam in increasing doses. At the end of 72 hours, all of the embryos were extracted from eggs and they underwent pathological examination with hematoxylin cosine and immuno-histopathological examinations with CD138 and tubulin beta II.
RESULTS: While the groups A and B showed no neural tube defects, totally eight defective embryos were detected in the groups C and D (three in group C and five in group D.
CONCLUSION: Our results suggested that meloxicam, a nonselective COX inhibitor, caused neural tube closure defects when injected at supratherapeutic doses. However, further studies with larger numbers of subjects are needed for its use in lower doses.

Abstract
Trigeminal neuralgia is a syndrome due to dysfunctional hyperactivity of the trigeminal nerve, and is characterized by a sudden, usually unilateral, recurrent lancinating pain arising from one or more divisions of the nerve. The most accepted pathogenetic mechanism for trigeminal neuralgia is compression of the nerve at its dorsal root entry zone or in its distal course. In this paper, we report four cases with trigeminal neuralgia due to an unknown mechanism after an intracranial intervention. The onset of trigeminal neuralgia after surgical interventions that are unrelated to the trigeminal nerve suggests that in patients with greater individual susceptibility, nerve contact with the vascular structure due to postoperative pressure and changes in cerebrospinal fluid flow may cause the onset of pain.

Abstract
Introduction: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy caused by the compression of the median nerve in the carpal tunnel in the wrist. In the diagnosis of CTS, neurological examination and electrophysiological studies (EMG) findings are based primarily. This study was conducted in order to determine how effective magnetic resonance imaging examining in the diagnosis and follow-up of patients diagnosed with carpal tunnel syndrome is.
Material and methods: Our research was performed on 21 patients with 23 wrists diagnosed with CTS with clinical and electrophysiological tests. Magnetic resonance imaging (MRI) was performed preoperatively and postoperatively in 14.2 weeks for the hands of 21 patients in the anatomical prone position by using a wrist coil. The length and width measurements of the sections of the median nerve were proportionalized at three levels (length / width), and flattening ratio of the median nerve, median nerve area, the intensity of the median nerve were evaluated. All patients was operated by the same surgeon, neurolysis was not applied to any patient.
Results: When the results of the EMG before and after surgery of the study group were analyzed, it has been detected that, there was a significant improvement in motor conduction velocity and sensory conduction time compared to preoperative results. Hand / wrist MRI of the study group was compared with the control group, it has been observed that, the arithmetic mean of all measurements, except the ratio of carpal tunnel area / wrist area were greater than the control group. The differences in terms of median nerve flattening ratio in the middle and distal, the distal median nerve area, flexor retinaculum thickness, flexor retinaculum spring, the flexor retinaculum spring ratio were not significant.
Conclusion: Electrophysiological studies are the gold standard in the diagnosis of CTS. MRI is an effective method in the evaluation of patients after surgery. It may be used to reveal the cause of failed surgery. MR imaging is an effective method in the diagnosis of patients and in post-operative follow-up.

Split cord malformations are classified as type I and type II and can be associated with lipomyelomeningocele. Although multiple lipoma in different levels at the same patient has been described, combination of SCM with double lipoma at the same level in literature is only once described previously. Herein, authors presented a 12 years old girl who had SCM type II associated with double lipoma at the same level.

Abstract
Fibroblast growth factor (FGF) plays a role in the development of bone and cartilage. FGF-2 is a member of this family and blocking of FGF-2 affects the bone development. In this study, effects of FGF-2 blocking on the formation of vertebrae in chick embryos before ossifications and orientation of cervical vertebrae were investigated with histological examinations. In this study, anti-FGF-2 was performed to chick embryos at E4th and E10th days. Samples were taken in E15th days and changes in the cervical spine were evaluated as histochemically (with hematoxylin-eosin, Alizarin red, Masson's trichrome staining) and immunohistochemically (with iNOS, eNOS and TUNEL). Also changings were evaluated by morphometric analysis. In macroscopic examination of the cervical vertebrae significant difference was not detected. But in histochemical staining, defects were observed in cartilage and ossification process. Increasing in oxidative stress was demonstrated by iNOS and eNOS. Also apoptosis which was represented by tunnel was found to be increased. FGF-2 blocking affects the ossification process in the cervical spine by inducing oxidative stress and apoptosis which results with cell death.

Abstract
Shunt application is one of the most common treatment method for hydrocephalus. Many complications have been reported in literature for ventricular and peritoneal catheters and shunt valve. Spontaneous knotting of peritoneal catheter is an extremely rare condition. Mechanism of knotting is still unclear and peristaltism, elasticity of shunt materials, intraabdominal density, length of the peritoneal catheter and surgical technique may be responsible factors. In this paper, we presented a peritoneal catheter knotting resulted in shunt dysfunction in an 8-year-old girl. During surgery, we removed the peritoneal catheter, untied the knot and placed in the peritoneum again.

Abstract
AIM: We describe a modified form of traditional open surgery with a new guide tube.This guide tube permits anterior screwing of odontoid in a shorter time with a more simple technique as compared to traditional open surgery, endoscopic and percutaneous surgeries.
MATERIAL and METHODS: Our series includes 6 patients who were operated for unstable odontoid fracture. We used a new guide tube for anterior odontoid screw fixation. This guide tube was designed by the first author to facilitate the insertion of the K-wire for placement of a cannulated lag screw.
RESULTS: Successful placement of the odontoid screws and immediate spinal stabilization were achieved in all patients. Solid fusion was observed during follow-up time in all patients.
CONCLUSION: This screw insertion technique for odontoid screw fixation provides a minimally invasive, safe and easy surgery in contrast to other surgical approaches.

Abstract
Objective The aim of this study is to demonstrate the process of detaching neural and surface ectodermal layers soon after the neurulation completes.
Materials and methods Specific pathogen-free chicken egg embryos were used to investigate the neurulation procedure. Ten eggs were saved as controls. The other ten eggs were opened at the 30th hour of embryo development and cultured with Z-VAD-FMK (peptide caspase inhibitor) to investigate the results of the apoptosis inhibition. Embryos were placed and developed up to 48 h in the culture medium. To detect apoptotic cells between neural and surface dermal layers, immunoreactivity of p53 and terminal uridine deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay were used.
Results While the control group shows positive immunoreactivity of p53 and TUNEL-positive apoptotic cells at the site where the neural folds detach from the surface ectoderm, no TUNEL activity and no detachment were detected in the apoptosis-inhibited group.
Conclusion As inhibition of apoptosis prevented the detachment of the neural and surface ectodermal layers from each other at the end of the neurulation, inhibition of apoptosis seemed to cause a considerable embryological error accounted for congenital dermal sinus tractus maldevelopment.

Abstract
The causes of tethered spinal cord are various. In order to release the tethering effect of these malformations, the surgical interventions must include removal of all tethering components, reconstruction of the neural tube and sectioning of tight filum terminale as well.
The cases reported in this paper have had an operation many years before for various developmental defects. After a certain period of time (5-10 years) of the first operation, the patients reapplied to the hospital with various symptoms of spinal cord tethering, either vertical or horizontal.
At surgical intervention, it was noted that inappropriate surgical procedures caused retethering of the spinal cord in all patients. Postoperative period of all patients were uneventful. All patients declared relief in their symptoms. We would like to draw attention that untreated (or inappropriately treated) midline developmental defects will invariably cause syndrome of tethered cord. Consequently, prophylactic surgical untethering must be applied to all patients with developmental midline defects as soon as possible.
It looks that tethered cord symptoms invariably appear as enough negative influence accumulates as the time passes. Elapsed time may vary but unpleasant end result invariably arrives. While these cases with tethered spinal cord develop progressive neurological symptoms, prophylactic and appropriate surgical intervention should be considered as early as possible. There is no acceptable rationale to wait for the appearance of tethered cord syndrome symptoms to perform surgical untethering of the spinal cord because of the probability of irreversibility of the symptoms (incontinence of urine in particular) of tethered spinal cord syndrome.

Abstract
The aim of this study is to elucidate the preventive effect of folic acid (FA) on teratogenic effects of valporic acid (VA) in early stage chick embryos on neural tube development.
One hundred and fifty specific pathogen-free (SPF) chick eggs were used to investigate the neurulation in five groups. Group A was the control group. Group B was injected 0.02 ml of saline (0.9% NaCl) and was used for sham group. VA (0.72 mg) in 0.02 ml saline was injected in Group C, and 0.342 mcg of FA in 0.02 ml NaCl were administered to the embryos in Group D. VA (0.72 mg) + 0.342 mcg of FA in 0.02 ml saline were administered simultaneously to the eggs in Group E. At the end of 72 h, all embryos were extracted from eggs and were fixed, and for histological analyses hematoxylin and eosine was used, for detection of apoptotic cells terminal deoxyribonucleotide transferase-mediated dUTP-X nick end labeling (TUNEL) was used and for distribution of P53, bcl-2 and caspase-3, caspase-6, caspase-8 and caspase-9 immunoperoxidase techniques were used.
While there were no neural tube defects in the embryos of groups A, B and D, eight embryos died in group C and there were 12 embryos with retarded embryological development. In contrast to that, no death was observed in group E, but only eight embryos were detected with maldevelopmental delay stage.
These results suggested that VA may induce apoptotic mechanisms but not through the p53 pathway. In addition, FA effectively prevents the teratogenic influence of VA on chick embryo at neurulation stages by stopping cascade of apoptosis before caspase 3 expression.

Abstract
Purpose: Cerebral vasospasm is the leading cause of morbidity and mortality following subarachnoid hemorrhage. Although a number of factors have been examined in clinical and experimental studies, the agent(s) responsible for developing and diminishing vasospasm remain poorly understood. Here, the role of edaravone, an antioxidant agent, was evaluated for its ability to diminish vasospasm in an animal model of subarachnoid hemorrhage.
Materials and Methods: A rat basilar artery subarachnoid hemorrhage model was used. Rats were divided into three groups: sham (n=7; Group 1), subarachnoid hemorrhage (n=7 Group 2), and subarachnoid hemorrhage plus edaravone (4 mg/kg intraperitoneally, n=7; Group 3). At the end of the seventh day, the rats were sacrificed, their brains were removed, and sections were taken from the basilar artery. These were examined using a light microscope, comparing the internal luminal circumference of the basilar artery of each group.
Results: The circumference was largest in Group 1, followed by Group 3 and then Group 2. That of Group 3 was 2% higher than that of Group 2, but this difference was not statistically significant.
Conclusion: This animal model for vasospasm suggests that edaravone helps enlarge internal luminal circumference following vasospasm caused by subarachnoid hemorrhage. It may do this by blocking lipid peroxidation and thereby reducing the effects of oxyhemoglobin and reactive oxygen species.

Abstract
A 47-year-old female patient who experienced urinary urgency after having stretching exercises of her legs is presented. Stretching of the legs are thought to be responsible for conus medullaris tethering which in turn causes urinary complaints. What is interesting in this case is that her complaints about urinary urgency stopped as she quit stretching exercise movements of her legs. There are some examples of such cases in the literature. The urinary urgency after stretching exercises warned us to investigate this patient in regard to tethered conus medullaris. Tethering of the conus medullaris can be temporary and can cause reversible functional disorders of the bladder. Incontinence at any age should be evaluated cautiously as it could be a sign of an underlying important developmental failure.

Abstract
A case of 19 years old female patient with 24 large primary cerebral hydatid cysts, in different localisations is presented. Cysts were removed unruptured by five separate operations in 6 months. The patient was observed to be in good health 1 year after the last operation. This case is the first case presented that contains such high number of cysts to our knowledge.
The importance of MRI examination in localizing the cysts and designing the operative plan is emphasized. Interestingly, albendazole, which is an antiparasitic drug, was reported to be ineffective by infectious disease department of our hospital, because of large diameter of the cysts, in this case. Abstract
This study aims to determine the dysfunction caused by existing pathological condition in structures involved in the transfer of sensory functions of the neural system in cervical disc herniation, and to establish whether or not the level and degree of this anatomical damage can be anticipated by SEP (Somatosensorial Evoked Potentials).
We compared the obtained SEP values for statistical significance using the Friedman Variation Analysis. In parameters with statistical significance, the Wilcoxon Signed Rank test was used to identify when significant improvements occurred.
The study found that the statistical data of the latency of the N14 wave originating from the dorsal column nuclei of the medulla spinalis and dorsal column gray matter improved (p<0.05) in the postoperative period compared with the preoperative values. Using the Wilcoxon Signed Rank test, we studied postoperative months separately in regard to the difference in the latency of the N14 wave, and found the statistically significant improvement to be marked particularly in months 3 and 6 postoperatively (p<0.05).
In conclusion, we suggest that SEP is a useful tool to check the functional condition of the dorsal spinal column. The benefit of the SEP utilization is the ability to determine the severity of the pathological condition preoperatively and follow the patient's functional postoperative improvement.

Abstract
Background. Intramedullary spinal cord metastases presenting as the first manifestation of malignancies are extremely rare lesions. Case Description. The authors report a 74-year-old woman who presented with an isolated intramedullary spinal cord metastasis which presents as first manifestation of malignancy without central nervous system and/or other organ involvement. She went under surgery, and after histopathological evaluation, primary focus was determined in lung in positron emission tomography. She is still alive after 9 months since the first diagnosis of primary focus. Conclusion. In patients with solitary intramedullary lesion in the spinal magnetic resonance imaging, whole-body investigation might help for diagnosis of primary focus and approach to treatment.

Abstract
The families of TGF-beta s and Smads proteins that controls its intracellular signaling pathways are known to play a role in early neurulation. The aim of this study is to demonstrate distribution of TGF-beta s (1, 2, 3) and Smads (1/2/3, 6, 7) proteins as a system in different hours of neural tube development of chick embryos. The SPF eggs were incubated at 37.8 +/- 2 degrees C for 24(th), 30(th), 48(th), 72(nd) h. After that, embryos were examined using immunohistochemistry and western blotting techniques. To the results, TGF-beta s immunoreactivities (particularly TGF-beta 3) at the 24(th), 30(th) and 48(th) h of chick development (during neural tube closure) were determined and decreased at the 72nd h (after neural tube closure), but expressions of TGF-beta s were detected in all stage of embryos in western blotting. While Smad 1/2/3 immunoreactivitiy and expression was less than that of the Smad 6 and 7 at the 24(th), it was increased at the 30(th) h. Smads proteins immunoreactivities were decresead at the 72(nd) h. In conclusion, the members of TGF-beta s are play a role in chick neural tube closure, the secretions of TGF-beta s are controlled different Smad proteins. In addition, immunoblotting results showed that TGF-beta s and Smads proteins were effective in the development of all tissues and organs of the embryos.

Abstract
Object This paper reports four tethered cord cases who initially applied with lumbar disk disease symptoms.
Materials and methods All of them were investigated by magnetic resonance imaging (MRI) and somatosensory evoked potential (SSPE). In two patients, MRI revealed thick and fatty filum terminale, while in remaining two, the filum terminale appeared as normal in thickness. SSEP revealed pathological conduction values in all, and it was the main indicator for surgical sectioning in patients especially with normal MRI investigations. All patients had benefit from the surgical sectioning of the fila terminalia with either thick and fatty or normal appearance.
Conclusion A particular patient with lumbar disc disease symptoms having normal MRI should also be investigated for tight filum terminale. If there is no pathological appearance in MRI investigation (both in regard to disc herniation and thick and fatty filum terminale), SSEP investigation should be done to check whether a conduction block or delay is present, indicating cord tethering.

Abstract
Connective tissue defects may play a significant role in the development of intracranial aneurysms (IAs). Multiorgan connective tissue disorders may, therefore, indicate a risk of IA development. We investigated biomarkers of connective tissue disease in patients with IAs. A series of 62 patients with IAs was studied by physical examination, echocardiography, ultrasound examination of the kidneys and abdomen, and microscopic examination of skin tissue (temporal area) and superficial temporal artery taken at operation. Patients with IAs had a higher incidence of biomarkers of systemic connective tissue disease than controls and identification of these markers may be important for screening for IAs. Microscopic investigation of biopsies of the skin and superficial temporal artery from patients and their relatives may become valuable for clinical diagnosis, identification of people at risk and basic studies of the pathogenesis of this vascular disease. Abstract
Objective: There is a growing interest in cystic lesions of the brain. Examining the cyst content of brain tumors may contribute in determining the malignancy of the given tumor accompanied by a cyst.
Methods: In this work, samples of cyst fluid from 18 patients with brain tumor were collected and studied biochemically regarding their protein, lactate contents and pH values; magnetic resonance spectroscopic images of these patients were also compared. We investigated the relation between the grade of malignancy and the lactate concentration and the discrepancy between the high levels of lactate in cysts and their alkaline environment.
Results: There appears to be a positive relation between the grade of malignancy and the concentration of lactate in the cysts' fluid. A significant two-fold increase in lactate concentration in malignant tumors cysts has been found as compared with the more benign tumor cysts (p<0.001). This increase in lactate level is probably because of aerobic glycolysis, which causes lactate production by the tumor.
Discussion: High lactate levels found through magnetic resonance spectroscopy are positively related to the grade of tumor malignancy. The pH values in the cyst fluids were above normal, resulting to a discrepancy in high levels of lactate in the cyst and the alkaline environment. This suggests efflux of H+ ions by a Na/H exchange mechanism to compensate for the change of pH.

Abstract
Objectives: To investigate acute changes of systemic parameters following experimental subarachnoid hemorrhage (SAH) and cerebral vasospasm conditions created by intracisternal injection of fresh autologous blood in a rabbit model.
Methods: The study was carried out at the Neurosurgery Clinic, Izmir Training and Research Hospital, Izmir, Turkey between April 2002 and March 2005. Thirty-two male rabbits were divided into the following 4 groups: group one with mild hemorrhage received 0.5 cc of blood, group 2 with moderate hemorrhage received 0.7 cc of blood, group 3 with severe hemorrhage received 1 cc of blood, and the sham-operated group 4 with no blood. The clinical parameters of all animals were monitored 2 hours after the operation, and blood analysis was performed just after hemorrhage. A series of systemic parameters such as rectal temperature, systolic and diastolic blood pressure, and blood gas analysis were measured before and after administration of blood injection.
Results were compared by analysis of variance and paired t-tests, and p-values less than 0.05 were considered significant. Results: The systolic blood pressures were significantly different in groups 1, 2, and 3 before and after the bleeding period. The diastolic blood pressures were also considerably different.
Conclusions: This study suggests that the presence of a blood clot and different amounts of blood in the subarachnoid space can evoke a series of changes in both local and systemic states in experimental models.

Abstract
Spina bifida is the one of the most frequently occurring birth defects. More children have spina bifida than muscular dystrophy, multiple sclerosis, and cystic fibrosis combined. Occulta present type is a mild very common form of spina bifida. The reported frequency of occurrence of spina bifida occulta (SBO) varies widely, depending largely on the age groups included in a particular study. The most accurate estimate of occurrence rate is 17% of examined spines. In present study we have examined and compared two Turkish siblings with spina bifida occulta who had neurosurgical intervention. Clinical, histopathological and cytogenetical analyses had been performed on mother and both siblings (brother and sister) diagnosed with spina bifida occulta. Hypertrichosis on their low-backs was diagnosed in both siblings. Magnetic resonance imaging (MRI) studies revealed low conus medullaris and thick filum terminale in brother and sister. The brother somatosensory evoked potential (SEP) results showed lumbar conduction blockade which was not found in sister. Despite the brothers's thick and fatty filum terminale sisters's seemed to be normal. Filum terminale sections from both siblings had normal appearance but atypical structure with increased amount of connective tissue and hyalinization areas. Sections from 9 years old boy were also prominent in glial cells. Genetical analysis revealed normal caryotype in 13 years old sister (46,XX), however deletions on chromosome 17 have been detected in 9 year-old brother [46,XY/46,XY,del(17)(q25)/47,+mar] and their mother [46,XX/46, XX,del(17)(q25)]. Our results show on strong correlation between the deletion of chromosome 17(q25) with genetical and histological results in both siblings with SBO. This is the first report of chromosome 17 (q25) deletion related to the SBO and its genetic connection with neural tube defects.
